THIS FORM MUST BE READ AND SIGNED BEFORE PARTICIPATION IN ANY BREEZY PETALS CAMPING RESORT LTD EVENT

In consideration of the services and/or equipment provided by Breezy Petals Camping Resort Ltd. for laser tag and associated activities, I understand and agree that Breezy Petals Camping Resort Ltd., its owner(s), directors, officers, employees, referees, independent contractors, contractors, volunteers, agents or representatives, successors and assigns (collectively referred to as "Breezy Petals Camping Resort Ltd.") are not responsible for any injury, loss, or damage of any kind that may occur as a result of my participation in laser tag, caused in any manner, including negligence, whether active or passive.

**ASSUMPTION OF RISKS IN LASER TAG:**

I am aware that participating in laser tag involves risks such as physical injury, tripping, failing, physical contact, over-exertion, and emotional effects. I acknowledge and assume all these risks, including the risk of severe or fatal injury.

**FITNESS TO PARTICIPATE:**

I confirm that I am in good health and suffer from no conditions that would make me especially susceptible to injury, disability, or death while participating in laser tag.

**RELEASE OF LIABILITY, WAIVER OF CLAIMS & INDEMNIFICATION:**

In consideration of participating in laser tag, I :
- Waive any claims against Breezy Petals Camping Resort Ltd. for any loss, damage, expense, or injury.
- Assume all risks associated with laser tag, even if caused by Breezy Petals Camping Resort Ltd.
- Hold harmless and indemnify Breezy Petals Camping Resort Ltd. from any liability for personal injury or property damage to any third party arising from my participation.

**GENERAL TERMS:**

This agreement is binding upon my heirs and assigns. If any provision is invalid, the remaining provisions remain in effect. This agreement is governed by the laws of the Province of Nova Scotia.

I HAVE READ THIS DOCUMENT, FULLY UNDERSTAND ITS TERMS, AND VOLUNTARILY AGREE TO THE PROVISIONS OF THIS WAIVER AND RELEASE OF LIABILITY.

FOR PARTICIPANTS UNDER 18 YEARS: I, as the parent/legal guardian, grant permission for the Minor to participate in laser tag, and I agree to the terms outlined in this agreement, waiving any legal rights.

By accepting, you consent to the use of your electronic signature. You have the right to request a paper copy but waive that right. You may withdraw consent in writing, with no penalty.
